Notes for Missing Class or Work

it is each student’s responsibility to meet all course requirements that are set by his/her professors. if a student feels that he or she is too ill to attend class, take an exam, or complete an assignment, the student must notify the professor directly. it is up to the professor to decide if he or she will accept this personal report of illness as a valid excuse.

Health Services will not2020欧洲杯体育直播投注 formally excuse a student from class with a written note, except in the case of serious illness or public health concern. Health Services will, with the student’s written permission, verify the date of a student’s visit to our office as well as the extent of his or her illness, should the professor or teaching assistant choose to contact us for this information.


we cannot provide documentation related to covid-19 for clearance for travel, work, or class attendance. we also cannot provide documentation for those requesting exemptions from wearing face coverings. we cannot provide documentation for those seeking accommodations related to the covid-19 pandemic unless specific criteria are met.

Parental Consent for Treatment

If a student is younger than 18 years of age when they arrive on campus, a parent or guardian must provide written consent for treatment. Students under 18 cannot access health care from Health Services without a completed Parental Consent Form on file. This form must be completed if the student is under 18 years of age at the time of arrival on campus, even if the student will turn 18 during the academic year.

Information About Students Over the Age of 18

2020欧洲杯体育直播投注in accordance with hipaa (health insurance portability and accountability act), if a patient is 18 years of age or older we cannot release any medical information to a parent, spouse, instructor, employer or anyone else other than the patient him/herself. patients who would like health services to verify an appointment or discuss diagnosis and treatment with anyone other than themselves must sign an authorization consent form during their visit. this consent pertains to that day’s appointment only; a new release form must be signed at each visit.

Confidential and Voluntary HIV Testing: New York State Public Health Law

ADHD Diagnosis and Medication Refills

students diagnosed with attention-deficit / hyperactivity disorder (adhd) who would like to have their stimulant medication refilled by a provider at health services must provide our office with adequate documentation of an adhd diagnosis. these records must be comprehensive, authoritative documents not more than three years old and prepared by a licensed professional in psychiatry, neurology or psychology. diagnostic reports should be on professional letterhead with name, address, phone and professional credentials of each evaluator as well as date(s) of testing. all quantitative information should be reported in standard scores and/or percentiles and must be based on age norms.

2020欧洲杯体育直播投注this information should be faxed to 716-829-2564 in advance of your appointment. we cannot accept documentation from family practitioners or school psychologists, as they are not ordinarily qualified to conduct the assessments we require. the prescribing of stimulant medications is at the sole discretion of the providers at health services. we reserve the right to decline to prescribe stimulant medications should we feel the diagnosis has not been established, the student needs more specialized treatment or there are adherence and/or abuse concerns.

Appointments: Eligibility, Frequency and Tardiness

2020欧洲杯体育直播投注health services offers health care services to any registered ub student. the vast majority of our services are paid for by the students’ mandatory health fee.  if you are a ub student who is currently registered for classes, you can be seen regardless of the type of health insurance plan you have. you do not need to be insured through one of ub’s health insurance plans to utilize health services.

Please note: visiting scholars, OPT, faculty, staff or the general public are not eligible for services through Health Services. We recommend contacting your insurance company to find health care provider options in Western New York.  (Typically, contact information for your insurance company can be found on the back of your insurance ID card.)

One Appointment Per Day

patients are limited to one appointment per day with a physician, nurse practitioner or physician assistant.

Late Arrival

2020欧洲杯体育直播投注if you arrive 10 minutes or more late for your scheduled appointment, you may be rescheduled for a future date and time. when you are late, it causes other scheduled appointments to be delayed, which is not fair to our other patients. please try to arrive 10 to 15 minutes before your scheduled appointment to make sure you have plenty of time to find parking (if needed), check in for your appointment and complete any necessary paperwork.
